Application Forms

As is common for specialty markets, ICC uses its own supplemental applications since:

-   ACORD apps don’t give us all the information we need to properly underwrite your business

-   We need to properly identify which of our different underwriters will do the best job for

     your restaurant and answer their questions.

Therefore, our application philosophy is:

• Complete the normal ACORD apps for property & liability.

-   ACORD 125 – General Information

-   ACORD 126 – General Liability

-   ACORD 140 – Property

and if applicable, the following:

-   ACORD 127 – Automobile

-   ACORD 141 – Crime

-   ACORD 131 – Umbrella

-   ACORD 155 – Boiler & Machinery

•   Complete the On-Screen Supplemental application for each location
  
  (if this is a franchise with many locations with the same characteristics, the Location Supplemental can be done once with notations of any departures)
